Duplicate or Corrected Form W-2 Frequently Asked Questions

If you need a duplicate W-2 for the current tax year, please allow a reasonable delivery period from the date we first mail the originals.

If you did not receive your original Form W-2, or if you received it but misplaced it, contact the Customer Support Center directly.

In most cases, the Customer Support Center will mail the duplicate to you within 24 working hours of receiving the request.

You may also print a copy of your W-2 through Employee Express.

If you want the duplicate W-2 sent to an address other than the one to which it was originally mailed, you must make a written and signed request. Mail or fax the request directly to Customer Support Center at the following address:

Include your name (as it appears on your LES), your Social Security Number (last 4 digits only), your employer agency/organization, and a daytime telephone number to reach you in case we need additional information. Also, include the statement that you need a duplicate Form W-2 issued, your old and new mailing address, and the applicable tax year. Be sure to sign the request.

If you need a duplicate W-2 after April 15 of the current year, request it directly from the Customer Support Center by mail, fax, or telephone. Include your name (as it appears on your LES), your Social Security Number (last 4 digits only), your employer agency/organization, and a daytime telephone number to reach you in case we need additional information. Also, include the statement that you need a duplicate Form W-2 issued and the applicable tax year. Be sure to sign your request.

If you believe the tax or wage information shown on your W-2 is incorrect, request a corrected one directly from POD by phone or fax. Include your name (as it appears on your LES), your Social Security Number (last 4 digits only), your employer agency/organization, a daytime telephone number to reach you in case we need additional information, and the year in question. Also, include the reason you believe the original W-2 is in error. Please be sure to sign and date your request.
