Current and Proposed Organizational Charts

If your organization is planning a reorganization or realignment, this page shows examples of the Current and Proposed Organizational Charts that must be completed for the reorg package to be complete. Follow the process outlined below to complete all required information for both the current and proposed organizational charts. Both charts must be provided and must fit on one page. They should include the following criteria:

  1. Layout of hierarchy of offices.
  2. Supervisor name labeled for each office.
  3. Org code name labeled for each office (same name indicated on the Reorg Crosswalk sheet).
  4. Org code number labeled for each office (same as indicated on the Reorg Crosswalk sheet).
  5. For IBC clients Only: FBMS cost center code labeled for each office (same as indicated on the Reorg Crosswalk sheet).
